University St. Anne’s University Hospital Brno provides comprehensive diagnostic, medical and nursing care, institutional, outpatient and stationary and performs comprehensive pharmacy activities. Designated Departments of the Faculty Hospital at St. Anne Brno are the teaching bases of the of Masaryk University’s Faculty of Medicine.

An application for information pursuant to Act No. 106/1999 Coll., On free access to information may be filed by any natural or legal person. The application may be made orally or in writing.

The oral application is filed with the Legal Affairs and Personnel Department, Pekařská 53, Building L (tel. +420 543 183 281 or +420 543 183 283). An applicant may be asked to make an application in writing when he or she is asked to do so.

Written application can be submitted:

  • by letter to the address of the St. Anne’s University Hospital in Brno, Department of Legal Affairs and Personnel, Pekařská 53, 656 91 Brno

The application is made on the day when the St. Anne’s University Hospital in Brno (FN) received it.
The request for information must be clear in:

  • the designation of the FN as the subject to which it is addressed
  • that it is a request for information according to the law on free access to information
  • name and surname, date of birth and residence of a natural person,
    the name, identification number and registered office of the legal entity

If the applicant fails to provide information on the applicant under the previous paragraph of the application, the FN shall ask the applicant within 7 days from the date of filing the application to complete the application. If the applicant fails to comply with this request within 30 days from the date of delivery, the FN will postpone the request.

If the request is incomprehensible, it is unclear what information is required or formulated too broadly, the FN will ask the applicant within 7 days from the date of filing the application to clarify the request. If the applicant fails to specify the application within 30 days, the FN decides to reject the application.

In the event that the required information does not fall within the scope of the FN, the request will be postponed with justification and FN will notify the applicant within 7 days of the request.

If the request for information is intended to provide information that may be publicly available at any time (the information disclosed), the FN shall provide the applicant with information within 7 days of the request making it possible to retrieve and obtain the information disclosed. If the applicant insists on providing the information directly, the FN will provide it.

The FN will provide the requested information within 15 days of receipt of the application or its completion (clarification). The deadline for providing information may be extended for important reasons, but not more than 10 days. The applicant shall be informed in good time and provably of the extension of the time limit including the reasons.

If requests for information can not be fully or partially met for legitimate reasons (eg, protection of personal data, business secrets, protection of classified information), the FN decides within 15 days of filing an application for refusal or part thereof.

The appeal to the application rejection must be made in writing. The appeal is made on the same contacts as the request for information. The FN will hand over the appeal within 15 days to the Ministry of Health, which will decide on the appeal within 15 days.

The complaint procedure may be filed in writing or orally in order to process the request for information. If the complaint is made orally and can not be processed immediately, the FN writes a written record. Complaints may be filed within the time limit and for reasons provided for by the Freedom of Information Act. The complaint is decided by the Ministry of Health.
